Referrals for orthopaedic consultation can be faxed to 416 636-9119. Please include the patient’s name, date of birth, OHIP number, patient’s contact information and the reason for the referral.

The office will contact the patient directly with the time and location for the appointment.

When seeing Dr. Zarnett for a knee problem, please bring shorts and for a shoulder problem a T-shirt or tank top.

Please bring copies of any imaging including x-ray reports, MRI reports and/or ultrasound reports.

The consultation may take place at Humber River Hospital, Fracture Clinic (Main Floor, Portal of Care “D”) or at the Cleveland Clinic Sports Medicine - 150 Eglinton Avenue East, please indicate on the referral if you have a preferential location for the consultation.
